Request for Water Meter Replacement due to Meter Lost

Consumer(s) who is/are facing water meter lost due theft can apply for water meter replacement.
The applicant is required to provide the following supporting document :
  1. Proof of Identity (MyKad or Passport or Company Registration Certificate)
  2. Authorised Letter from owner if application is by representative
  3. Police Report

Legal owner or representative with letter of consent.
The applicant is required to pay the meter cost.
The applicant is require to settle the following charges :
  1. Applicant is required to pay bill for new water meter replacement.
  2. Installation fees of new water meter.
