Traveling During Covid-19 Movement Control Order?

      

Health Declaration Form   Permission Form to Enter/Exit Sarawak

  1. Travellers entering Sarawak at any point of entry are required to complete this mandatory declaration form. Failure to do so would result in a denial of entry to the State.
  2. Travellers are fully aware that it is a criminal offence to induce the State Government of Sarawak to grant entry on the basis of a false declaration;
  3. The data provided by travellers is completely confidential and only serves for the prevention of epidemics, under the management of the State Disaster Management Committee.

  2. Flight ticket / itinerary
  3. Proof of working or employment
  4. Proof of marriage
  5. Invitation from Sarawak company or individual including trip purpose
